PROTOCOL ON ENTRY OF FOREIGN APPLICANTS

I. Definition of Foreign Applicants
Foreign applicants are those who are nationals of a country other than the Philippines, holding a foreign passport and who have been issued a valid student visa or Special Study Permit or Permanent Residency Visa by the appropriate Philippine authority. The term "foreign applicants" includes those who claim dual citizenship.

II. Application Requirements for Foreign Applicants
The application requirements for foreign applicants to be submitted in two (2) sets to the Office of International and External Affairs (OIEA) for initial evaluation are:

  • Application Form duly accomplished
  • Six (6) copies of original Personal History Statement (PHS) duly accomplished and signed by the applicant in English, accompanied by personal seal, if any, original left and right hand prints on the PHS and original recent colored photo 2"x 2" with white background.
  • A notarized Affidavit of Support and Proof of Adequate Financial Support to cover expenses for the applicant's accommodation and subsistence, as well as school dues and other incidental expenses. (i.e. Bank Statement)
  • Passport pages where name, photo, birth date and birth place appear
  • Student Visa duly issued by the Philippine Embassy or Consulate in the applicant's country of origin or legal residence.(upon arrival and for foreign applicants in the Philippines)
  • Scholastic Records (Transcript of Records) duly authenticated (not seen) by the Philippine Foreign Service Post (PFSP) located in the legal residence or country of origin.
  • Birth Certificate duly authenticated (not seen) by the PFSP.
  • Certified photocopy of Diploma, duly authenticated (not seen) by the PFSP.
  • Police Clearance certificates issued by the National Police Authorities duly authenticated by the PFSP in the applicant's country of origin. NBI Clearance for those applicants who resided in the Philippines for more than 59 days.
  • Certificates of Good Moral Character from two (2) professors in previous school/university.
  • National Medical Admission Test (NMAT) result. Inquire from the PFSP for the place and date of the test. (for medical student applicants)

III. Application Procedure for Foreign Applicants

A. Foreign Applicant Abroad

  • The foreign applicant communicates to OIEA Office his/her intention to study with UPHS.
  • He/she submits the documents enumerated above to IEA Office for initial evaluation.
  • OIEA shall serve as the clearinghouse of all matters concerning entry and pre-registration of foreign student within a particular program
  • Upon submission of the required documents, the OIEA endorses the foreign applicant together with his/her documents to the Admission Office for the Entrance Exam.
  • Upon compliance, the Admission Office shall endorse the foreign applicant to the Campus Registrar for the issuance of the Notice of Acceptance (NOA).
  • The Registrar submits the documents to the Department of Foreign Affairs (DFA)
  • The DFA endorses the documents to the PFSP in the applicant's country of origin or legal residence for the issuance of the corresponding visa.
  • PFSP notifies the applicant with instruction that he/she appears in person before the Consular Officer for interview and submission of required documents (NOA, Police Clearance and Medical Health Certificate).
  • The OIEA shall manage the arrival of the foreign applicant in the country and in the campus.
  • Upon arrival, the OIEA shall endorse the foreign applicant to the Admission Office and the Registrar to assist his/her registration to the program.
  • The Registrar assist the foreign applicant in securing Alien Certificate of Registration (ACR) and Certificate of Residence for Temporary Students from the Bureau of Immigration and Deportation (BID)
  • The respective campus Registrar shall submit subsequent reporting and submission of required reports to CHED and BID as scheduled
  • Upon enrollment of the foreign student, he/she shall be under the care and supervision of the School Director throughout his/her stay in the University.

B. Foreign Applicants in the Philippines

  • The foreign applicant shall proceed to IEA Office and submits the documents enumerated above for initial evaluation.
  • OIEA shall review the documents submitted by the foreign applicant.
  • Upon compliance of the required documents, the OIEA shall endorse the foreign applicant to the Admission Office for the Entrance Exam.
  • Upon compliance, the Admission Office and the Registrar shall assist the foreign applicant in his/her enrollment in the desired program.
  • The campus registrar shall assist the foreign student in securing Alien Certificate of Registration (ACR) and Certificate of Residence for Temporary Students from the Bureau pf Immigration and Deportation (BID).
  • The respective campus Registrar submits required reports to CHED and BID as scheduled.
  • Upon enrollment of the foreign student, he/she shall be under the care and supervision of the School Director throughout his/her stay in the University

C. Foreign Applicant without Student Visa

  • Upon evaluation of OIEA that the foreign applicant failed to secure a Student Visa, Special Study Permit or Permanent Residency Visa, OIEA shall endorse the foreign applicant to the campus Registrar for the issuance of the Notice of Acceptance (NOA).
  • The campus Registrar endorses to BID the request of the foreign applicant for the conversion of the status from temporary visitor to foreign student visa.
    -inclusive documents: Original copy of the Certificate of Eligibility for Admission (CEA) issued by the CHED, proof of adequate financial support, scholastic records and police clearance authenticated by the PSFP located in the applicant's country or origin or legal residence, quarantine medical clearance certificate, copy of PHS and copy of the valid Passport.
